The Food Establishment Inspection Report is a weekly report that appears Sundays. Information is taken from reports provided by the Environmental Health Department, and individual reports can be viewed at its website, amarillo.gov/departments/community-services/environmental-health/food-inspections. A numerical grading system is now being used, with a score of 100 points being equivalent to zero demerits.

• 100 points – No major violation at the time of inspection

• 85 points – Considered an average score

• 70 points or below – Requires re-inspection within 24 hours

• COS – Corrected on site during inspection

• (A/95) Affiliated Foods, 1401 W. Farmers Ave. Spoiled/damaged/recalled food must be placed in a designated area; heavy accumulation of ice on and inside packaging of pallet of corndogs. COS. All exterior doors must be sealed to prevent entry of pests; door in coupon reclamation area must be repaired to allow door to latch properly; food must be stored at least six inches above the floor; heavy accumulation of unknown liquid covering floors in dock area being spread by moving equipment causing splash onto watermelon and other packaging; nonfood-contact surfaces of equipment must be kept free of accumulation of dust, dirt, food residue and other debris; accumulation of debris in janitor’s closet of cold dock; coving coming loose from walls in employee restrooms in warehouse; no soap at employee hand sink. Correct by 09/04.

• (B/85) Beef Burger Barrel, 3102 Plains Blvd.  No sanitizer in food service area; chemicals must be stored below three-compartment sink. Correct by 06/09. A Certified Food Manager must be on premises at all times; no paper towels at hand sink; cigarette butt in hand sink. Correct by 06/16. Must have a designated area for smoking; wiping cloths must be stored in sanitizer solution between uses; nonfood-contact surfaces of equipment must be kept free of accumulation of dust, dirt, food residue and other debris; hand sink is separating from the wall and must be repaired; floor tiles missing; utility sink must be accessible for mop water disposal; bathroom door must be self-closing. Correct by 09/04.

• (A/98) Bosco’s Lounge, 2307 S.W. 6th Ave. Back door needs to be sealed to prevent entry of pests; walk-in by condenser unit is soiled with dust. Correct by 09/04.

• (A/98) Brady’s Dairy Queen, 2601 S. Soncy Road. Freezers near grill accumulating food debris and heavy ice build-up; heavy dust build-up on intake air ducts; dust build-up on fan guards in walk-in cooler; unshielded lights above three-compartment sink. Correct by 09/06.

• (A/97) Chill Natural Bar, 6200 Hillside Road, Space 100. Fruits stored in cardboard box in fridge beginning to grow mold. COS. Light mold beginning to grow inside ice maker. Correct by 09/06.

• (A/97) Circus Room, 2309 S.W. 6th Ave. Back door must be sealed to prevent entry of pests. COS. Sanitizer test strips needed. Correct by 06/16.

• (A/90) Eddie’s Napoli’s Italian Restaurant, 700 S. Taylor St. Clean silverware stored in dirty bins. Correct by 06/10. Quaternary and chlorine sanitizer test strips needed. Correct by 06/17. Back doors must be closed or screens installed if doors are open; wiping cloths must be stored in sanitizer solution between uses; iced tea and food stored uncovered; sauce in walk-in must be stored 6 inches off the floor; dishes stacked wet; bathroom doors must be self-closing. Correct by 09/05.

• (A/90) El Tropico, 2310 9th Ave., Canyon. Evidence of active pest infestation in establishment. Correct by 06/10. Allergen advisory must be posted; water from melting ice around coolant lines leaking and pooling on floor in food and consumer areas; ice machine must be cleaned and sanitized inside. Correct by 06/17. Ice scoop must be stored in a container that allows draining or must be stored on a clean and sanitized surface; Styrofoam cups must not be used as scoops; nonfood-contact surfaces of equipment must be kept free of accumulation of dust, dirt, food residue and other debris; surfaces in kitchen and bathroom areas where moisture is present must be constructed of non-absorbent material; wood base under water heater, sides of wooden lockers in restrooms and food storage areas and any other area where bare and exposed wood is found must be sealed or replaced to ensure surfaces are non-absorbent; water-damaged ceiling tiles in consumer area must be replaced. Correct by 09/05.

• (A/95) Fazoli’s, 2512 S. Soncy Road.  BBQ sauce requiring refrigeration after opening, stored on bottom shelf of prep table. COS. Weather stripping torn at bottom of outside door in dry storage area; uncovered items in coolers and next to pasta boiling station. Correct by 08/31.

• (A/92) Freddy’s Frozen Custard & Steak, 5225 S. Coulter St. Grilled onions at improper temperature. Correct by 06/09. Mold accumulating in ice maker; unlabeled containers holding sugar in dry storage; blood from patties in back cooler; intake and exhaust air ducts must be cleaned and filters changed; obnoxious odor coming from plumbing. Correct by 09/04.

• (A/98) Henk’s BBQ, 1508 S. Grand St. Soda machine leaking syrup. COS. Wiping cloths must be stored in sanitizer solution between uses. Correct by 09/01.

• (A/95) Jorge’s Mexican Bar & Grill, 6051 Bell St. Several pots and pans on shelves wet and holding water; exhaust air ducts accumulating dust. COS. Bottom of frozen Margarita machine leaking oil/lubricant; bowl used as scoop inside flour container; food buildup from previous night on drawer cooler under grill; floor in walk-in freezer beginning to detach from concrete floor underneath; hand washing signs needed at all hand sinks in kitchen and prep area. Correct by 09/04.

• (B/84) La Chiquita Bakery, 3500 E. Amarillo Blvd. No drip pan at vent hood in kitchen and grease is dripping down walls. Correct by 06/11. No Certified Food Manager; food handler certification needed; several items not date marked; wood on bottom trim in back room must be painted or stained to be smooth, durable and non-absorbent. Correct by 06/18. Mouse droppings near back door; both back doors and side doors must be sealed; food in walk-in not covered; several items in establishment stored directly on the floor; walls and inside of reach-in coolers soiled with debris; two light bulbs in vent hood burned out; outlet cover missing. Correct by 09/06.

• (A/99) McLellan Learning Center, 401 S. Van Buren St. Employee personal items must be stored in a designated area. Correct by 09/06.

• (A/97) Nobilitea, 6200 Hillside Road, Space 200. Dried syrup splatters on cooler and wall behind flavored syrups. COS. Dust collecting on top side of tea and cup dispensers; wall next to dump sink soiled and beginning to peel exposing drywall; Certified Food Manager certificate must be posted in view of customers. Correct by 09/06.

• (A/94) Olive Garden #1130, 4121 W. I-40. Carts stored in front of back hand sink and no soap at same hand sink. COS. Wiping cloths must be stored in sanitizer solution between uses; water splashing onto bulk food item containers and dripped into food when lids were opened; ROP (reduced oxygen packaged) fish thawing improperly; clean equipment and utensils must be stored in a position to allow for air drying; scoop handle touching flour. Correct by 09/05.

• (A/99) Panhandle Surgical Hospital, 7100 S.W. 9th Ave. Towels must not be used under items to keep dry. Correct by 09/01.

• (A/92) Sam’s Southern Eatery, 4317 Teckla Blvd. Dirty dishes stored on clean dish rack and in a bin full of dirty water. COS. No sanitizer solution in kitchen. Correct by 06/11. Prepared gumbo and other food items in walk-in cooler not date marked. Correct by 06/18. Old food build-up on shelves in walk-in, floor under equipment and reach-in coolers. Correct by 06/22. Back exit door needs new weather strip; numerous flies in kitchen; hot held foods by fryer without lids; filters and vent hood dripping grease. Correct by 09/06.

• (A/99)Schwan’s Home Service Truck #519021, 10300 W. UH 60, Canyon. Gaskets to freezer units on driver side must be repaired or replaced. Correct by 06/12.

• (A/94) South Dock Cafeteria, 1401 W. Farmers Ave. Food accumulation on potato slicer. COS. Wiping cloths must be stored in sanitizer solution between uses; nonfood-contact surfaces of equipment must be kept free of accumulation of dust, dirt, food residue and other debris; splash guard missing from sink in men’s restroom. Correct by 09/04.  

• (B/83) Subway #11143-326, 105 N. 23rd St., Canyon. Cold hold unit in food prep below warmers and microwaves not cooling at proper temperature; kitchen ware not being used must be stored in a place that does not accumulate dirt, food and other debris; knives and utensils on prep line heavily soiled and no sanitizer in knife sanitizing container; heavy accumulation of syrup and other food debris on vending spouts. Correct by 06/10. Certified Food Manager must be on site and CFM certificate must be posted in view of public; food handler certification needed; walk-in cooler leaking water from condenser; mold growing on non-functioning electronic thermometer in walk-in cooler. Correct by 06/17. Drinks and medicines stored next to single-serve items in drive-through area; knives used for slicing sandwiches must be stored clean and sanitized between uses; all non-food contact surfaces must be cleaned of any accumulated dirt, dust, food and other debris. Correct by 09/05.

• (A/97) Subway #14124-214, 5752 W. Amarillo Blvd.  Gap at bottom of back door; employee personal items must be stored in a designated area; underside of soda dispenser dirty with syrup build-up. Correct by 09/06.

• (A/95) Taco Bell #032695, 2000 S. Coulter St. Soda dispensers heavily soiled. Correct by 06/05. Paper cup used as scoop for bulk items; nonfood-contact surfaces of equipment must be kept free of accumulation of dust, dirt, food residue and other debris; heavy accumulation of dust on vent hood filters. Correct by 08/31.

• (A/99) The Hotdog Shack Mobile, 1415 Sunrise Drive. All wood surfaces must be sealed to be non-absorbent; ceiling needs to be sealed. Correct by 09/05.

• (A/98) Toot ‘n Totum #44, 5424 River Road. Flat grills need to be professionally steam cleaned; an organized system throughout the facility is needed for tools, chemicals and single use items. Correct by 08/31.

• (B/86) Washington Food Mart, 4016 S. Washington St. Out-of-date pre-packaged bologna, hot dog wieners, tea products and canned goods on shelves. COS. Hose connected to faucet at mop sink must have an anti-siphon/backflow device installed. Correct by 06/11. No sanitizer test strips; sign needed above hand sink indicating for hand washing only; sinks near vending and by three-compartment sink must be labeled and stocked; wood surfaces on shelving storing food products in walk-in coolers must be replaced with a non-wood surface. Correct by 06/18. Gap at back door must be sealed; front doors must be sealed; a hole in exterior fascia in rear of building causing birds to create a nest in ceiling crawlspace must be repaired; floor surfaces must be repaired or replaced where missing, broken or in need of repair (repeat violation); unsealed sheetrock/tape and bedding must be sealed; any cracks or openings to upper crawlspace must be sealed; employee restroom must have a non-absorbent wall/coating/tile or other non-absorbent surface installed a minimum of 4 feet in height from floor that encircles perimeter of the restroom; ceiling must be sealed/repaired to be non-absorbent (repeat violation). Correct by 09/06. 

• (A/99) Yolo’s Food Truck, 316 S. W. 6th Ave.  Nonfood-contact surfaces of equipment must be kept free of accumulation of dust, dirt, food residue and other debris. Correct by 09/05. 

• (B/81) Youngblood’s Café & Catering Co., 620 S.W. 16th Ave. Rust build-up in ice machine; can opener and vegetable slicer heavily soiled; chemicals stored on same shelf as food products. Correct by 06/11. All hand sinks being used for purposes other than hand washing; employee beverages must have a lid and straw; wiping cloths must be stored in sanitizer solution between uses; food stored uncovered in coolers; food stored directly on the floor in walk-in cooler; scoop handle touching flour; vent hood systems and walls need to be clean to sight and touch; light bulbs burned out in vent system in back kitchen; back area has flooding possibly coming in from the outside (check sealing and ensure there is no water standing in establishment).
